I affirm that I am in sound physical and emotional condition and able to participate in a rigorous movement and coaching program. I understand that participation in any session or activity offered by Azul Productions and TNM Coaching at the Rise Evolutionary Coaching & Embodied Awakening Retreat is voluntary on my part, and that there are inherent risks of accidents or injuries, which I assume for myself.
I am aware that body contact may be part of the movement sessions offered during the retreat and take full responsibility for establishing, maintaining, and appropriately expressing my own physical boundaries.
I acknowledge that the Azul Productions and TNM Coaching staff, faculty, and practitioners will not render any medical services or diagnoses of my physical or psychological condition. I take full responsibility for my participation in all activities I choose to undertake.
I agree that all activities I undertake during the retreat are entirely my own responsibility, and that Azul Productions and TNM Coaching shall not be liable for any damages or injuries to my person or property, including those resulting from acts of negligence on the part of retreat staff, faculty, or practitioners. Azul Productions and TNM Coaching shall also not be responsible or liable for any articles lost, stolen, or damaged on the premises.
In consideration of my acceptance as a participant in such activities, I expressly waive, release, and hold harmless Azul Productions, LLC and TNM Coaching Ltd, their officers, employees, faculty, and successors, from any obligations, liabilities, claims, demands, costs, or expenses, during or after my participation in the retreat.
I further agree that Azul Productions and TNM Coaching make no express warranty that the programs or sessions will have any particular effect or benefit.
This waiver shall be governed under Hawaii law in respect of Azul Productions, LLC, and under the laws of England and Wales in respect of TNM Coaching LLP.
